INSURANCE

I am a out-of-network provider for all insurance carriers.  You will be given a receipt for the services provided to submit to your insurance company with all necessary information.

I encourage all patients to contact their insurance companies to discuss their out-of-network coverage, as many plans provide significant reimbursements of treatment costs. Insurance that includes out-of-network benefits often reimburses 20-90% of fees.
